Lake Health achieves 100 percent core measure compliance

Lake Health, a two-hospital system in Lake County, Ohio, encompasses more than 30 ambulatory sites, including four urgent care centers, physician offices, walk-in centers, physical therapy, diagnostics, and a free-standing emergency room. Lake Health uses workflow technology embedded in Soarian® Clinicals to help its clinical team adhere to the Joint Commission Core Measures for Pneumococcal and Influenza Vaccinations.

Lake Health’s Soarian Clinicals Implementation Team (SCIT) comprises nurses, pharmacists, respiratory therapists, and dietary and clinical informatics professionals. Recognizing the importance of information technology in improving care quality, the SCIT designed and implemented a vaccination administration workflow in 2010. Based on a subsequent analysis of this workflow, the SCIT identified some “lessons learned,” such as eliminating unnecessary process steps and duplicate documentation, and gathering input from front-line staff. Lake Health launched a revised vaccination administration workflow in 2011.

At the end of 2011, Lake Health had achieved 100 percent compliance with both the pneumococcal and influenza vaccine administrations for those patients who met the core measure criteria. This compares favorably to its baseline of 86 percent compliance in 2009.

Lake Health’s vaccination workflow includes a mandatory screening chapter as part of the patient’s admission assessment. The screening chapter helps the nurse identify any contraindications to the vaccine, such as patient refusal, inability to screen, physician indicating not to administer, or the patient has recently received the vaccine. If contraindication(s) exist, the screening tool indicates “do not give pneumococcal vaccine.” If no contraindications are documented the nurse is asked about the patient’s temperature. Depending on the answer, the nurse then receives support for appropriate medication administration, follow-up, and compliance with administration requirements.
